Our hospital school's transition procedures are designed to ensure comprehensive support for students moving between hospital and their regular school environments. We also nurture their next step in their educational pathway to further education and employment.
This process includes

The completion of a sharing of information and consent form to gain necessary permissions to liaise with stakeholders.

Sharing the student’s enrolment with their census school.

Admission and discharge emails are sent to inform census schools about when students arrive at our school and when they leave. Upon discharge, census schools receive a detailed learning record for each student, aligned with stage-appropriate curriculum content.

Facilitating the sharing of relevant student information with the census school.

Conducting a "Lesson Zero" session that incorporates a return to school form, a Tar3 calming and support plan, and a "Getting to Know You" form.

Collaboration occurs through regular attendance by executive staff to clinical and stakeholder meetings.

Daily Learning and Support Team (LST) procedures help assess and prioritise students’ needs around medical and learning needs.

Family meetings are held alongside clinical teams and independently as a school to ensure effective communication is had.
These procedures are guided by the principles of personalised learning and support and involve regular review of support strategies to address each student’s diverse learning and wellbeing needs.
